Changing your contact details

To update details like your name, address, phone numbers or email address:

•  fill out a change of details form 
• visit the surgery from 10am to 6pm

Change of address

If you have moved to a new address, you will need to be in our catchment are to sat registered with our surgery.

• check you are in our catchment area

Change your phone number or email address

You will need to show us identification before we change your phone number or email addresses.

Change your date of birth or name

You will need to show us proof that you have changed your name or date of birth.  This can be a marriage or civil partnership certificate, deed poll or birth certificate.
