New Patient Registration

The doctors welcome new patients who live within our practice area, see map below.

You are now able to register at Friarwood Surgery online through NHS UK:Online Registration form

Please note your online registration request will be pending until you receive a response from the Practice to confirm your registration. We aim to respond within 5 working days of your request being sent through.

Alternatively, please contact the surgery for the relevant forms or complete the forms below and hand them in at reception. Where possible please provide proof of address and photographic ID such as a passport or driving license when registering online. Children require a copy of their birth certificates showing parents names. 

Medical treatment is available from the date of registration. Please contact reception for further information.

Please ensure you add your 10 digit NHS number to the registration form, this is unique to the individual and can be found on a prescription counter foil, appointment letter, by asking your current surgery for it or by using the link below.
